We have an exciting opportunity in our Adult Family Care (AFC) Program in the Bristol County, MA area for a full time (35 hours per week) RN Case Manager.
The AFC Registered Nurse/Case Manager evaluates potential participants and foster families, monitors member/caregiver placement status, provides case management services, serves as liaison with assigned member’s primary care physicians, monitors member’s health status, completes semi-annual health status reports and patient care plans, implements and evaluates caregiver training sessions and assists in outreach to increase community awareness of AFC Program services.
The AFC Registered Nurse/Case Manager must be a Registered Nurse and must have two years of recent experience with elders and individuals with disabilities in a community setting. Must be currently MA licensed in good standing and CPR certified.

Hours are Monday through Friday, 8:30 a.m. – 4:00 p.m., with no weekend or holiday shifts.
